The Ember Codex
Coaching templates: every document of a coaching engagement, who fills it, and where it lives
A coaching engagement runs on about a dozen documents. Here is each one, who fills it in, where it can live, and the Codex page with its template.
A coaching engagement runs on about a dozen documents: agreement, intake form, coaching plan, session template, question bank, session plan, model worksheet, action plan, accountability record, program outline, somebody else’s form, session record.
Sorted by subject, a template library tells you what exists. Sorted by who fills each one in, it tells you which ones you can write once and which ones wait on another person.
Canva, Notion, Pinterest and The Coaching Tools Company held the first page on 2026-10-07, each grouping its library by subject. Copy the index below and fill it in.
| Document | When | Who fills it |
|---|---|---|
| Agreement | before session one | you draft, client signs |
| Intake form | before session one | client |
| Coaching plan | session one | you and the client |
| Session report | every session | you |
What do you need before session one?
Five documents, and the one the client fills in is the one that can hold up session one.

One sheet per model, and the client writes on it during the session.

What does the session itself run on?
Two before the call, one during it, one you set once.
| Document | Filled by | Lives in | Codex |
|---|---|---|---|
| Session outline | you, during and after | a notes doc | opening to commitments, fixed and flexing |
| Question bank | you, before | your list | not published yet |
| Session plan | you, before | your notes | not published yet |
| One template for both | you, once | your AI stack | every session type |
What has to exist between sessions?
All four hold what the client said in the room, and nothing here tracks what they do after it.
| Document | Filled by | Lives in | Codex |
|---|---|---|---|
| Action plan | the client commits, you record | a shared doc | the actions box |
| Accountability record | you | a platform | the record |
| The model, between sessions | you | your template | the model |
| The session with no commitment | you, still | the same template | none this session |
One r/lifecoaching poster put it this way: “It’s not that much about the tools, but about the systems you create for yourself.”
What is the record, and who keeps it?
The report exists because the session happened, so ask your client about recording and check your jurisdiction.
| Document | Filled by | Lives in | Codex |
|---|---|---|---|
| Session report | you, or your AI | your letterhead, PDF or Word | built box by box |
| Its prompts and pass mark | you, once | your template | must and never, the pass mark |

| A coaching platform | you | Paperbell, $970 a year; no documents listed, 2026-10-07 | platforms compared |
| Ember, closed beta | you correct it in the editor | your AI stack; Free covers 4 reports a month | letterhead and sections |
I build Ember, so read its row as a disclosure.
Build a template for every row whose middle column does not say you. Those are the ones that wait on somebody else.
Questions people ask next?
What are the 7 P’s of coaching?
There is no agreed list. ycoaching.com.au names Purpose, Perspective, Process, Presence, Preparation, Practice and Performance; SimplyBook.me gives the marketing seven instead; Coach & Coachee starts Presence, Pause, Poise. No document follows from any of them.
Where do the free PDF templates come from?
Carepatron’s coaching plan page offers a free printable PDF and Notion’s marketplace carries the GROW goals worksheet, both read 2026-10-07. craftedcharts sells its set instead, and free here means the blank form.
I build Ember, a session-report builder for coaches, and it is in closed beta. Free covers 4 reports a month; Individual is $8 a month billed yearly.
